File of Life form

What if you had a medical emergency in the middle of the night? 
When Emergency Medical Services personnel arrive at your home, they may have questions regarding the patient’s medical history, current medications, primary care providers etc.  During this stressful moment it could be a challenge for the patient or family members to provide this information. 

Your SIFD has a tool to help you organize this crucial information and have it accessible in the moment. 

The ‘File of Life’ form allows you to have all of this information in a readily accessible single format. 

We ask that you keep the form’s magnetic pouch on your refrigerator.  First Responders know that is where these forms are kept and are therefore easy to find.
